It has been designed to serve as a ready reference source for the council by providing the appropriate report forms to file and the dates that these reporting forms are due at the state office. The Supreme forms can be accessed on this page as well. These "Online" forms can be filled out, and electronically submitted to the Knights of Columbus.(Click on Title above to access the State Forms.) Supreme and State Forms should be submitted prior to their due dates, preferably as soon as all data are available.
